NHS West Kent want to improve the care they offer people at the end of their life,

This is a difficult subject to talk about and prepare for but it will happen to us all eventually.

So NHS West Kent would be very grateful if you would take a little time to think about dying and preparing for end of life, and who you would need supporting you, and how you might share those preferences, to make sure your views are known and respected.

Many people and services maybe able to support you at this difficult time if they know what your views are, so please tell us what you think and help us to improve the care we offer.
